Your Health Passport is the “off the top of your head” information a clinician asks you at the desk. You can fill it in before you arrive, then share it with any clinician, even one who isn’t on PHB.

What you can add

Open Records (/records). You can add and remove:
  • Allergies, conditions and medicines.
  • Immunisations.
  • Family history.
  • Past surgeries.
  • Lifestyle.
Items you add yourself carry a self-reported label. It means a clinician hasn’t confirmed them yet. The label shows on your Records page and in the shared view. Self-reported items never count as confirmed by a clinician.

Share it with a clinician

Sharing is time-limited, passphrase-protected and read-only, and you can revoke a link at any time. The steps are in Share with a clinician.

What the clinician sees

They open /share/… with no PHB account and enter the passphrase. The page is read-only and watermarked. It shows when it was generated, when the link expires, which view this is, and only the last four digits of your HPN. Self-reported items keep their label. The page also has Bring PHB to my hospital. It lets the clinician ask PHB to contact their facility. They give their own business contact, not your details.
Sharing may be tied to completing a National Identification Number (NIN) check in the future. This is switched off today and never affects the care you receive.
