# Independent pharmacy application

> How an independent pharmacy applies to join PHB, and what happens after.

An independent pharmacy, one that isn't part of a hospital, applies online. PHB checks the documents by hand.

## Before you start

You need:

* The pharmacy's name, state and LGA.
* The **CAC / RC number** and **PCN premises number**.
* The **Superintendent pharmacist**'s first name, surname, email and **PCN licence number**.
* Copies of the documents: CAC certificate, PCN premises licence, and the superintendent's PCN licence.

## Steps

    Go to `/pharmacy/apply`. The page is titled **Register your pharmacy on PHB**.

  <Step title="Your pharmacy">
    Fill in **Pharmacy name**, **State**, **Local Government Area**, **CAC / RC number** and **PCN premises number**.
  </Step>

  <Step title="Superintendent pharmacist">
    **First name**, **Surname**, **Email address** and **PCN licence number**. This is the superintendent's own practising licence.
  </Step>

  <Step title="Your documents">
    Upload the documents.
  </Step>

  <Step title="Submit">
    You see **Application received** and **Thank you — we have your application.**

## What happens next

1. A PHB administrator reviews the documents by hand.
2. If approved, PHB sets up the pharmacy and emails the superintendent a link to set their password.
3. If rejected, PHB emails the reason.
4. The superintendent signs in at `/pharmacy/login` and can invite more pharmacists.

  No public PHB page links to `/pharmacy/apply` yet. Share the address directly.

## Common problems

| Problem | What to do |
| - | - |
| No email after applying | Reviews are done by hand and can take time. Check spam. |
| Application rejected | Read the reason in the email, fix it, and apply again. |
