# Who viewed my record

> See when clinicians opened or added to your record, and what emergency access means.

## Before you start

You need to be signed in.

## Steps

1. Open **Consent** or **Records**.
2. Find the **Access log**.
3. Read the entries. Each one shows who acted and when, for example **Viewed your record**, **Opened your record on a treatment basis**, **Added a result**, **Added a note** or **Access blocked**.

## What the entries mean

| Entry | Meaning |
| - | - |
| **Viewed your record** | A clinician with your consent opened it. |
| **Viewed your record (treatment basis · non-sensitive)** | A clinician treating you opened the non-sensitive part. |
| **Opened your record on a treatment basis** | A clinician started treating you and opened limited access. You were notified. |
| **Access blocked** | Someone tried to open it without a basis and was stopped. |
| **Added a result**, **Added a note**, **Opened an encounter** | Someone added to your record. |

## Emergency access

If a clinician uses emergency access, you get a notification. Emergency access is recorded and reviewed by PHB.

## Common problems

| Problem | What to do |
| - | - |
| You don't recognise an entry | Contact PHB, and revoke any access you no longer want. |
