# Records and results

> Your visits, lab results, conditions, medicines and notes in one place.

## Before you start

You need a PHB account and to be signed in.

## Steps

    Tap **Records** (`/records`).

    **Overview** shows your health record. **Profile** shows your personal details.

  <Step title="Open a section">
    Sections include encounters (visits), labs, conditions, medications and notes.

You can go straight to labs at `/records?section=labs`.

## What you can do here

* Read your visits, results, conditions, medicines and notes.
* Add your own allergies and health history. Items you add yourself are marked as self-reported.
* See medicines a pharmacy recorded for you, labelled by where they came from.
* Download your record. See [Download your record](/guides/patient/download-your-record).
* Share it. See [Share with a clinician](/guides/patient/share-with-a-clinician).

## About lab results

* A result appears only once the lab has released it.
* Some results are given to you by your doctor first. Until then, they don't appear at all. Your doctor will talk to you and then release them.

  **Imaging** and **Documents** are not built yet. They will be empty.

## Common problems

| Problem | What to do |
| - | - |
| A test is missing | It may not be released yet, or your doctor may want to talk to you first. Ask your doctor. |
| Imaging or Documents is empty | These aren't built yet. |
| You see "denied" | Your account has no HPN linked. Contact PHB. |
