# Health Passport

> Add your own health details, label them as self-reported, and share them with a clinician through a link and passphrase.

Your Health Passport is the "off the top of your head" information a clinician asks you at the desk. You can fill it in before you arrive, then share it with any clinician, even one who isn't on PHB.

## What you can add

Open **Records** (`/records`). You can add and remove:

* Allergies, conditions and medicines.
* **Immunisations**.
* **Family history**.
* **Past surgeries**.
* **Lifestyle**.

Items you add yourself carry a **self-reported** label. It means a clinician hasn't confirmed them yet. The label shows on your **Records** page and in the shared view. Self-reported items never count as confirmed by a clinician.

## Share it with a clinician

Sharing is time-limited, passphrase-protected and read-only, and you can revoke a link at any time. The steps are in [Share with a clinician](/guides/patient/share-with-a-clinician).

## What the clinician sees

They open `/share/…` with no PHB account and enter the passphrase. The page is read-only and watermarked. It shows when it was generated, when the link expires, which view this is, and only the last four digits of your HPN. Self-reported items keep their label.

The page also has **Bring PHB to my hospital**. It lets the clinician ask PHB to contact their facility. They give their own business contact, not your details.

  Sharing may be tied to completing a National Identification Number (NIN) check in the future. This is switched off today and never affects the care you receive.
